Acne
In addition to conventional therapies such as topical and oral medications, there are also effective laser and light based treatments for acne.

– Omnilux Blue and Red Light
The latest state-of-the-art technology for treating acne is Omnilux Blue and Red Light. Omnilux treats acne by generating high-intensity light which activates natural chemicals found in P.acnes bacteria, the most common cause of inflammatory acne. When stimulated, these chemicals destroy the bacteria and the light stimulates the cells in the body that help destroy P.acnes and aid with the healing process. The blue light targets P. acnes and the red light helps calm the skin by reducing inflammation. The light is produced by Light Emitting Diodes or LEDs. Typically, 75% patients see significant improvement in their acne after four weeks of treatments. Treatment protocol consists of 8 painless treatments over 4 weeks.

– Photodynamic Therapy
Photodynamic Therapy (PDT) is treatments for acne by utilizing a photosensitizing chemical that is applied to the skin about an hour prior to the procedure. The blue light activates the chemical that is specifically absorbed by active old glands and the bacteria that produces acne. This results in a reduction in the oil glands and the destruction of the bacteria. These treatments also effectively reduce sun damage and pre-cancerous lesions known as actinic keratoses.

Acne Scars
Acne is a chronic inflammatory disorder of the sebaceous glands. In addition to blackheads and whiteheads, painful, cystic bumps can form. These bumps can often leave behind a residual brown mark and even a scar in some patients.

There are many different types of acne scars.
Ice pick scars are deep seated scars that tend to be very narrow. Rolling scars are wavy and shallow while boxcar scars are round and can be wide, but more superficial than ice pick scars. These different types of scars respond to different treatments including lasers, fillers, microdermabrasion, dermabrasion, chemical peels, subcision and punch excision.

Age/ Brown Spots
The Q-Switched Ruby 694 nm laser allows us to treat pigmented lesions such as sun spots (lentigines), seborrheic keratoses, age spots, and birthmarks. The wavelength of the Ruby laser is thought to be the most effective in treating age spots and other pigmented lesions. NY Medical Skin Solutions is one of the few centers in Manhattan to have this device. After a treatment, the lesions will become darker after the treatment and gradually crust off over the next 10 days or so. Many of the darker age or brown spots will respond after just one treatment whereas some fainter or lighter spots may require another treatment.

Variable pulsed light (VPLTM) is a revolutionary new technology used to rejuvenate skin that has become wrinkled and blotchy as a result of cumulative damage from exposure to the sun and simply from aging. VPL can effectively treat brown age and sun spots, broken blood vessels, redness, large pores and can improve fine lines and wrinkles through collagen stimulation on the face, neck, chest and hands. There is even a dedicated attachment that emits the wavelength to target Propionibacterium acnes, the bacteria responsible for producing acne . Treatments with VPL are non-invasive and usually done in a series of at least three sessions with minimal downtime. They can even be combined with other treatments such as chemical peels and laser genesis. The VPL allows for greater flexibility in treatments and a more customized approach than standard IPL (Intense pulsed light) devices due to a multitude of pulse variations that can be programmed.

Broken Blood Vessels/Rosacea
Small broken blood vessels on the face typically appear around the nose. Sun damage is thought to be the likely culprit in why they form. Once they form, they will not resolve on their own unless treated with lasers.

Rosacea is a chronic condition characterized by facial redness and swelling around the nose, mouth, chin, and forehead. Specific triggers of the condition include sun exposure, alcohol, extreme heat or cold, and spicy foods. The broken blood vessels and general facial redness in rosacea can be treated with lasers.
Using the pulsed dye Vbeam PerfectaTM laser we have more versatility in treating any and all vascular conditions such as rosacea, broken blood vessels, angiomas, port wines stains, diffuse redness, and even warts, some scars and early stretch marks! Depending on your lifestyle the treatment can be tailored so that there is minimal purpura (bruising) and therefore downtime involved.

Chemical Peels
NY Medical Skin Solutions has a variety of peels to address a multitude of skin issues including uneven complexion, fine lines, large pores, oily skin, acne, and sun-damage. Click here to learn more.

Cosmetic Fillers
Along with Botox and Dysport, the new generation of injectable fillers has revolutionized the way we address the aging process. Available in a variety of formulations, these biocompatible fillers offer tremendous versatility for restoring youthful volume and contouring the face.

Today’s facial fillers are longer lasting and are often comprised of substances found naturally in the human body, such as the hylauronic acid fillers Restylane, Perlane and Juvederm, or the calcium based Radiesse.

A helpful metaphor for understanding the different types of fillers is to think of them as peanut butter and jelly. Peanut butter has a thicker consistency and is well suited for adding volume and sculpting areas of the face (contouring the cheeks, chin or nose) whereas jelly is lighter and more malleable making it a perfect choice for filling and smoothing lines and wrinkles.

Cosmetic Fillers Can Address Issues Such As

  • Glabella lines
  • Nasolabial folds
  • Laugh lines
  • Sunken cheeks
  • Thinning lips
  • Marionette lines
  • Scars and deep wrinkles
  • Facial Asymmetry

Dr. Saini is one of the few dermatologists selected to serve as national trainers for the cosmetic fillers Juvederm® and Radiesse®

Laser Genesis
Laser Genesis is a gentle, soothing, no downtime, “lunctime” procedure for non-ablative skin rejuvenation. There is no bruising or pain occurring after treatments, just some mild redness which may last an hour or so. Laser Genesis stimulates the production of collagen thru gentle heating of the middle dermis in the skin. This results in a subtle decrease in fine lines and wrinkles, reduces pore size, redness associated with rosacea and small dilated blood vessels, and improves the overall texture of the skin. It has also been shown to improve mild scarring in the setting of acne. You will notice a more youthful glow, smoother texture in your skin, and a decrease in pore size after a series of treatments. Laser Genesis can be used in all skin types safely and effectively. Often, a series of treatments are necessary, especially in the setting of redness-associated conditions such as rosacea in which they may be combined with treatments targeting individual blood vessels.

Laser Hair Removal
Unwanted hair can be bothersome for both men and women. Many modalities used to treat unwanted hair are often temporary.

Laser hair removal is also a very cost-effective way to treat unwanted hair. A recent study found that women spend up to $23,000 and 58 days of their lives removing unwanted hair using methods such as shaving, waxing and depilatory creams. Studies show that on average, women can save $7,000 by receiving laser hair removal on the areas of the body they typically shave and $20,000 by receiving laser hair removal on the areas of the body they usually wax.

Laser hair removal works by targeting the melanin, or pigment in the hair follicle. Therefore, black and brown hairs respond far better than lighter colored hairs.

In order to superiorly treat patients of all skin types we have the 755 nm Alexandrite device GentleLaseTM laser. This allows us to treat finer hair on lighter skin patients more effectively. Most patients notice a reduction by around 80% after four treatments.

NYMSS has the first-ever approved laser to permanently reduce hair in patients of every skin type. Prior to the development of this laser, laser hair reduction was too risky for tanned or dark skinned patients. Using the Coolglide Vantage Nd:YAG 1064 nm, we can safely and effectively reduce hair on the face, underarms, arms, bikini area, legs, just about anywhere in ALL patients! After applying a topical anesthetic 15-30 minutes beforehand, treatments are performed at setting appropriate for your skin type and hair thickness and texture. Because hair growth is in cycles, three to six treatments are often required to see significant results. Finally be done with shaving, waxing, plucking, and bleaching!

Latisse
Eyelash hypotrichosis is another name for having inadequate or not enough eyelashes.

LATISSE® (bimatoprost ophthalmic solution) 0.03% is a prescription treatment for hypotrichosis used to grow eyelashes, making them longer, thicker, and darker.

Microdermabrasion
Combining gentle abrasion and suction, microdermabrasion utilizes a spray of tiny crystals to remove the outer layer of dead skin. As with chemical peels, this process of exfoliation stimulates collagen and offers a gentle approach to lightening age spots, treating acne, and promoting a youthful glow. For optimal results, please contact us at NY Medical Skin Solutions to experience a series of treatments that alternate the use of chemical peels and microdermabrasion.

  • Improves sun damaged skin
  • Reduces acne scarring and hyperpigmentation
  • Extracts blackheads and whiteheads
  • Reduces age spots
  • Improves oily skin

Spider Veins
While it is not entirely clear what causes spider veins, heredity, pregnancy and hormones are likely major contributing factors. Up to 40% of women are plagued by varicose/spider veins. Sclerotherapy is the gold standard for treatment of spider veins. It is performed by injecting a safe sclerosing material directly into the blood vessels with the intent of collapsing them. We will decide which sclerosing material should be used and whether or not it should be foamed to increase effectiveness based on the size and nature of the vessels to be treated. Compression stockings must be worn for a period of time to improve results and help minimize transient side effects such as hyperpigmentation. Typically, an average of 3-4 treatments is required and some veins may not respond to treatment.

Using the 1064 Nd:YAG laser, you can have small dilated blood vessels around the face and legs as well as even deeper blue veins in the legs as well as other vascular conditions such as angiomas and port wine stains removed safely and effectively. Topical anesthesia is usually not used because it tends to constrict the blood vessels, making treatments less effective. Following the procedure, you may notice some redness and swelling that persists for a couple of days. In the case of leg vein treatments, compression stockings must be worn for a period of time immediately after the procedure. This laser treatment can be used in lieu of sclerotherapy in patients who prefer less invasive treatments as well as in conjunction with sclerotherapy to treat non-responsive veins.

Sun Damage and Wrinkles
As a person ages, their skin loses elasticity, becomes drier, and thinner. Wrinkles subsequently form and exposure from the sun, chemicals (such as smoking) and even normal facial expressions can expedite the development of wrinkles.

Wrinkles can be mild, thought of as fine lines, or deeper, ingrained in the skin, appearing more cavernous, such as the folds around the mouth.

Treatments for wrinkles and sun damage include lasers and light-based treatments, Botox, fillers, and chemical peels.
